We take all Motor Vehicle Insurance coverage and Workman's Compensation

We bill In Network for:

  • All Motor Vehicle / PIP claims in Oregon

  • Blue Cross / Blue Shield / Regence

  • MODA

  • Pacific Source

  • CareOregon OHP (follow up visits require authorization)

  • Anthem

  • Aetna

  • Work Comp

Out Of Network: If we are NOT contracted with your insurances, we are able to bill OUT OF NETWORK which often covers the visits. Be aware that you will need to be know your out- of -network coverage, which sometimes is subject to a deductible.

Superbills: We also provide a superbill if you would like to submit the bill to your insurance company for reimbursement of a visit that you paid for out of pocket. The front desk is happy to answer questions you may have regarding this service.

Gift Certificates: We offer packages and gift certificates for Low Level Light Therapy, Massage and Personal Training.

Insurance Benefit Check Form

Many people have chiropractic coverage under their insurance. Before you come in for your first appointment, as a courtesy to you, we will gladly research your insurance coverage; however, you will ultimately will be responsible to know your insurance coverage. You will also be responsible for full payment of your account if your insurance company does not extend coverage for our services. This form will need to be submitted at least 24 hours in advance of your treatment.
